Accountable Care Organization


Under the Affordable Care Act, The Centers for Medicare and Medicaid Services (CMS) finalized new rules to help physicians, hospitals, and other service providers give improved care for Medicare Fee-for-Service patients through Accountable Care Organizations (ACOs).

ACOs create incentives for healthcare providers to work together to treat an individual Medicare Fee-for-Service patient across care settings-including physician offices, hospitals and long-term care facilities.

HealthCare Provider ACO, Inc. is a network of health care providers.

HealthCare Provider ACO, Inc. is committed to protecting your personally-identifiable health information (PHI). Our Privacy Notice describes how we may use and disclose PHI. It also describes your rights as well as any significant obligations of HealthCare Provider ACO, Inc. regarding the use and disclosure of PHI.  The law requires HealthCare Provider ACO, Inc. to maintain the privacy of PHI, to provide this notice of HealthCare Provider ACO, Inc.'s legal duties and privacy practices with respect to PHI, and to follow the terms of this Notice that are currently in effect.

 This Notice applies to any individually identifiable information that HealthCare Provider ACO, Inc. obtains from its beneficiaries or others that relates to past, present and future physical or mental health, the health care its beneficiaries received, or payment for health care. Sources of this information include the Centers for Medicare and Medicaid Services as well as the independent physician practices that comprise HealthCare Provider ACO, Inc. Each of these providers is required to have its own Notice of Privacy Practices, and may share PHI for the purposes of treatment, payment and health care operations, including measurement of the quality of care rendered to its ACO beneficiaries.
